'contents' => '<p style="text-align: right;"> 【2005年08月22日】<br> 社団法人日本麻酔科学会<br> 理事長 武田 純三 </p> <p> 看護師とのかかわりについては、本年2月に社団法人 日本麻酔科学会が公表した「麻酔科医マンパワー不足に対する日本麻酔科学会の提言」において、看護師との協力と、その教育・育成に学会が関与すべきことが盛込まれている（注1）。<br> <br> 米国でのNurse anesthetistをはじめとして、類似の制度が世界各国で運用されていたり、議論されたりしている12）。日本でも"麻酔看護師"について議論されているが、十分に定義されないままで使用されていることが多く、誤解と混乱を招いている。<br> <br> 米国でのNurse anesthetistは、エーテルドームでの麻酔実験が成功し、エーテル麻酔が始まった頃より看護師による麻酔が開始されている歴史があり、教育も大学卒業後2-4年間行われているなど、しっかりした教育プログラムに則った制度である。<br> <br> 現在の"麻酔"は、安全性が非常に高いが、一旦イベントが発生すると患者の生命に直結するリスクの高い医療行為でもあり、麻酔科を専門とする医師以外の医師が積極的に関与しない傾向にある。また、国民皆保険の制度の恩恵に預っている現在、医師以外の職種に生命を委ねることは、今の日本にはなじまないと考えられる。<br> <br> そこで学会としては、目標として"麻酔は麻酔科専門医が行うべき"との姿勢を"提言"で明確にしている（注2）。また、看護師が麻酔業務に麻酔科医とともに関与し，より患者の安全性を高める役割を担うことは周術期の患者にとって有用なことと考えている。そのため麻酔科学会は日本看護協会と話し合いを持ちながら、麻酔を含めた周術期に関わる専門看護師を教育する方向で、業務内容と教育プログラム作成の検討を始めている。既に一部の施設では行われている業務も含まれるが、専門性を高めることで一層安全性、質の向上が図れるものと考える。<br> <br> 注1 <br> 「麻酔中の患者管理は麻酔科医と看護師が共同で行うことが望ましく、業務を分担することでお互いの業務の軽減につながり、また患者の安全面からも複数で監視することになり、より有用である。」<br> このように、麻酔に関して医師でなくとも可能な業務が存在しており、これらの業務を移行することでかなりの時間の有効利用ができる。そのために麻酔を介助する看護師の育成、手術室における薬剤師、臨床工学技士の配置が必要である。<br> また、看護師や臨床工学技師の教育は患者の安全のためにも必要なことであり、日本麻酔科学会が中心となって教育システムを作成し、育成に関与すべきである。」<br> <br> 注2<br> 「原則として麻酔は麻酔専門医が行うべきである。麻酔関連事故などの医療事故分析や、骨髄移植のドナーには麻酔科専門医による麻酔が求められているなど社会的要請からも、麻酔科専門医の資格を有している麻酔科医による麻酔が望まれる。」<br> <br> 文献<br> 1.
